I hereby acknowledge that my dog is a senior dog, and waive all age – related issues in regards to my dog while walking, boarding or spending the day with Rachel or any Dogs on the Run employee.

I consent to Rachel or any Dogs on the Run employee making any medical decisions along with either our dogs’ regular veterinarian, or any emergency veterinarian she chooses.

I understand that I will be contacted ASAP should there be any medical/cause for concern issues with my dog while in the care of Dogs On The Run.
